After reading about a second ND today in TN, you have to wonder "Why did this happen and how?"
I'm not going to armchair QB either incident, but I will tell you to negate it. Too many cooks in the kitchen.
There has to be ONE centralized collection point for all persons and equipment.
ONE PERSON - The same person from here on out and no others. (Except the second set of eyes that will shadow him or her)
ONE person to clear all weapons and equipment (Magazines, duty belts, etc) of anything lethal.
ONE person to inspect all those participating in the drill for the same on or about their person.
ONE person to inspect all the training equipment and gear to be used.
ONE person to issue all the training equipment and gear to be used.
ONE holding area for all personnel to remain once the training equipment and gear has been re-issued, until the scenario begins. And the same to be used where all personnel will return to be accounted for, once the scenario is MC.
A SECOND PERSON to remain with the ONE PERSON, from start until finish, in order to oversee and double check all actions and safety checks performed in order to double tap it for surety.
The Riggers Creed is a simple one and one that can be applied here - "I WILL BE SURE ALWAYS"
Check, check and re-check if necessary. These incidents, tragic or otherwise, do not need to happen to anyone, and can be prevented, including ones own self, each and everyday.
