First time it has happened to me. I didn't think it was anything nefarious, just annoying. Especially when it said "hey here's your picture" and it just... wasn't.

I’ve had it happen before, especially if it generates anything you download before seeing (eg update a text file). Sometimes the file is a blank/nearly-blank file with the correct name and nothing else.
