Drownings of 2 Navy SEALs knocked overboard in raid of ship with Iranian weapons were preventable: probe

It’s a tough read if you follow the timelines and detail the sailor went through as they fell, with everything with in the story though, I cannot get past this paragraph:

Flotation equipment that was properly maintained, working well and used correctly would have been able to keep them afloat until they were rescued, the report said.

Other team members told investigators that while they knew the importance of their tactical flotation system — which includes two inflatable floats that attach to a belt and foam inserts that can be added — few had ever operated one in training and there is little instruction on how to wear it.
 
But allegations over a lack of oversight in NSW are completely unfounded and isolated cases? The NSW community does not have a culture with a lack of integrity? Critics, including some on this forum, have questioned this for years and been shouted down by NSW and bro vets.

I went to a 5-day conference several years ago about the nature of this kind of thing, specifically as it applies to medicine. The industry doesn't matter, all the rules apply the same. Medicine adopted safety standards from the nuclear and aviation industry. It's all about risk, how much risk you assume, and the safety you think you have by doing the wrong thing many times over time and it turning out okay. It sounds like these guys got burned because of lax safety procedures.
 
...find it funny that a transgender sailor can get Testosterone treatments, but a straight guy gets booted.

It's because a straight guy taking steroids is cheating to make his muscles stronger which gives him an unfair advantage - a transgendered Solder, Sailor, Marine, or Airman is only trying to make her dick bigger...
...which is a victimless crime
